I found a copy of a 1913 1040 form and it turned out to be only four pages including the instructions. This really emphasizes how a tax once instituted will grow and become increasingly complex over time.

The thing that surpised me the most was how much this "simple idea" had already been complicated at that time and as we all know things are far more complicated now.  Just imagine how much more fun and expensive this "simple tax" will be in another 20 years...  That's why I do not favor taxes at all.
